PHYSICAL FORMS
Every child in the Forsyth County School District is required to have a current physical on file if they want to try-out and/or participate in an athletic program.
Physicals are current for one year from the date of the physical.
All forms are required!!!

Insurance Coverage
Sports physicals are $40, excluding lab work.
The players/families can make an appointment on our website (www.advanceduca.com) by selecting the Cumming office at any time. When registering online, an option for “Sports Physical” will be listed on the drop-down menu. They can pick a date and time that best fits their schedule.
